RN Case Manager Remote Compact License Required

Job Summary

A company is looking for a RN Case Manager with a compact license for a remote position.

Key Responsibilities:

• Complete post discharge assessments for members' clinical and social needs
• Educate members on disease processes and medications, manage and update care plans
• Collaborate with healthcare providers and departments, work independently following program processes

Required Qualifications:

• Current, unrestricted RN compact license
• 3+ years of experience in hospital, acute care, or direct care setting
• Basic level of experience with Microsoft Office
• Bachelor's Degree preferred, Certified Case Manager (CCM) a plus
• Designated workspace with high-speed internet access required
