Job Summary

A company is looking for an RN Case Manager for a remote contract position.

Key Responsibilities:

Lead a multidisciplinary team to deliver holistic care management to a diverse health plan population
Assess, develop, implement, monitor, and evaluate care plans to optimize member's health
Coordinate care delivery, advocate for members, and provide education on health management

Required Qualifications:

Nursing Diploma or Associates degree in nursing
3 years of clinical nursing experience in various settings
1 year of case management experience in a managed care setting
Current, active, and unrestricted Registered Nurse license (Compact License)
Certification in Case Management (CCM) required or to be obtained within 18 months of hire
