RN Case Manager - State Licensed

Job Summary

A company is looking for a Care Ally, RN Case Manager.

Key Responsibilities

• Conducts comprehensive health assessments and develops care plans for members
• Acts as a health coach, educating members and caregivers about treatment options and resources
• Coordinates communication and care among members, their families, and the interdisciplinary care team

Required Qualifications

• One (1) year of clinical practice experience in case management, home health, or related fields
• Proficiency in Microsoft Office applications and ability to navigate multiple systems
• Active and unencumbered Registered Nurse license or Licensed Social Worker (LSW) certification required
• Bachelor's degree in social work (BSW) preferred for LSW candidates
• Case management certification is preferred
