Job Summary

A company is looking for an RN Case Manager for Transitions of Care.

Key Responsibilities:

• Engaging with newly discharged members telephonically to assess needs and develop care plans
• Coordinating post-treatment care, referrals, and collaborating with healthcare providers
• Educating members on chronic conditions and providing active care management

Required Qualifications:

• Unrestricted compact RN license in the United States
• Minimum of 4 years of RN experience in case management or home health
• Desired experience in health insurance, telephonic care, or acute care
