Job Summary

A company is looking for a Clinical Care Specialist.

Key Responsibilities

• Coordinate and implement Chronic Care Management services for patients with chronic conditions
• Monitor patient health metrics and conduct care coordination with patients and their care teams
• Provide education on chronic condition management and support training for the care team

Required Qualifications

• Registered Nurse (RN) with an active compact license
• Five years of nursing experience required
• Experience with chronically ill patients preferred
• Telephonic case management or care coordination experience preferred
• Strong computer skills and reliable internet connection required
