Job Summary

A company is looking for a Clinical Care Specialist.

Key Responsibilities

• Manage and monitor a patient census, coordinating Chronic Care Management services for chronically ill patients
• Create individualized Care Plans and track patient progress, including labs, vitals, and medications
• Conduct care coordination and provide patient education while collaborating with the care team and families

Required Qualifications

• Registered Nurse (RN) with an active compact license
• Five years of nursing experience required
• Strong computer skills and reliable home internet connection
• Experience with chronically ill patients and telephonic case management preferred
• Self-motivated and independent worker
