RN Complex Case Manager

Job Summary

A company is looking for a Complex Case Manager III to provide member-centric care and coordinate services across the healthcare continuum.

Key Responsibilities

• Conduct comprehensive assessments and develop individualized care plans for members
• Collaborate with members, families, caregivers, providers, and interdisciplinary teams to deliver holistic care
• Monitor and coordinate services to reduce illness burden and healthcare costs for individuals and families

Required Qualifications

• Bachelor's degree in Nursing or equivalent work experience, plus 5 years of clinical/acute care nursing experience
• Current, unrestricted RN license in the state of residence
• Experience working with vulnerable and complex populations in clinical, home care, or telephonic environments
• Knowledge of managed care principles and regulatory guidelines
• Proficiency in electronic health records and care management software
