RN Complex Case Manager

Job Summary

Providing member-centric care in a remote setting, the full-time RN Complex Case Manager will manage a portfolio of high-need clients, coordinating services and resources to ensure safe transitions within the healthcare system.

Key responsibilities

• Conduct individualized assessments to identify members' goals of care and coordinate necessary services
• Support members in navigating the complexities of the healthcare system and connect them with community resources
• Collaborate with healthcare providers to ensure effective communication and continuity of care

Required qualifications

• Associate's or Bachelor's degree in Nursing
• 5+ years of clinical/acute care experience beyond degree
• Current, unrestricted RN license in the state of residence
• Certified Case Manager (CCM) preferred, or commitment to obtain within two years of hire
• Experience working with vulnerable and complex populations in a clinical or telephonic environment
