RN Discharge Care Manager

Job Summary

A company is looking for a Discharge Care Management Nurse RN Remote in Central or Eastern Time Zone.

Key Responsibilities

• Evaluate member discharge needs and develop readmission prevention plans
• Coordinate care post-discharge with providers and members
• Document discharge activities and participate in team meetings and education discussions

Required Qualifications

• Associates or Bachelor of Science in Nursing
• Current, unrestricted Compact RN license in the State of residence
• 5+ years of recent experience in the inpatient acute setting
• 2+ years of experience in discharge planning
• Experience with multiple health insurance products within the insurance industry
