Job Summary
Providing telephonic education and coaching, the full-time RN Health Coach will engage patients managing chronic conditions, supporting them in achieving healthier lifestyles while working remotely with a flexible schedule.
Key responsibilities
- Identify and engage patients who could benefit from coaching
- Conduct telephonic outreach to explain the program and encourage enrollment
- Educate and motivate patients toward healthier behaviors using motivational interviewing techniques
Required qualifications
- Active, unrestricted RN license in the US
- Previous experience in an employee wellness or health promotion role preferred
- Spanish-speaking ability is a plus
- Minimum internet speed requirement: 25 Mbps download / 25 Mbps upload
