RN Licensed Care Transition Specialist

Job Summary

A company is looking for a Care Transition Specialist to support their Outcomes Department.

Key Responsibilities

• Conduct post-discharge phone surveys and complete assessments in the database
• Verify patient demographics and discharge information for accurate documentation
• Document patient-reported concerns and escalate unresolved issues according to protocols

Required Qualifications

• Active, unrestricted RN or LPN license, or previous medical/clinical training
• Associate degree (or higher) in Nursing or a related clinical field
• Proficiency in medical terminology and ability to produce accurate clinical documentation
• Experience in planning and accomplishing goals while managing multiple tasks
• Strong listening and customer-service skills with a patient-centered approach
