RN Manager of Transitions of Care

Job Summary

A company is looking for a Transitions of Care RN Manager to lead a team of RN Case Managers in delivering exceptional care and patient outcomes.

Key Responsibilities

• Coach and support RN Case Managers through various methods including 1:1 meetings and audit reviews
• Identify workflow challenges and contribute to standardizing best practices across teams
• Develop onboarding and ongoing education experiences for staff while collaborating with cross-functional leaders

Required Qualifications

• Active RN license (compact/multi-state licensure preferred)
• Minimum 2 years of direct patient care experience in care management or transitions of care
• Minimum 2 years of people management experience with a focus on team development
• Experience managing licensed care management teams is preferred
• Proven ability to drive clinical and operational outcomes through initiatives and data-informed strategies
