Job Summary
A company is looking for a Transitions of Care RN Manager to lead a team of RN Case Managers in delivering exceptional care and improving patient outcomes.
Key Responsibilities
- Coach and support RN Case Managers through various methods including meetings and shadowing
- Identify workflow challenges and contribute to standardizing best practices across teams
- Develop onboarding and ongoing education experiences for staff while collaborating with cross-functional leaders
Required Qualifications
- Active RN license (compact/multi-state licensure preferred)
- Minimum 2 years of direct patient care experience in care management or transitions of care
- Minimum 2 years of people management experience with a focus on team development
- Experience managing licensed care management teams is preferred
- Proven ability to drive clinical and operational outcomes through new initiatives
