Job Summary

A company is looking for a Nurse Care Manager for the Eastern Time Zone.

Key Responsibilities

• Manage overall care for an assigned patient panel, tailoring care to individual needs and goals
• Establish trusting relationships with patients and families, providing clinical and emotional support
• Coordinate interdisciplinary care plans and monitor the effectiveness of care management strategies

Required Qualifications

• Associate degree in nursing
• Current RN License is required; Compact License preferred
• Care management experience is required
• Certified Case Manager preferred
• Intermediate skills with MS Office Suite, including Outlook and Teams
