RN Nurse Case Manager

Job Summary

A company is looking for a Nurse Case Manager, who will support patients through assessment, planning, facilitation, and advocacy in a remote setting.

Key Responsibilities

• Utilize assessment, planning, facilitation, and advocacy to coordinate patient care
• Ensure services align with benefit plans and meet patient health needs
• Collaborate with members, providers, and care teams to deliver coordinated care

Required Qualifications

• High School Diploma or equivalent; Bachelor's Degree preferred
• RN with compact unrestricted active license
• 2-3 years of clinical practice experience in healthcare settings
• 2 years of experience in the healthcare and/or managed care industry
• Case Management experience preferred
