RN Pregnancy Case Manager

Job Summary

Providing member-centered care, the full-time remote RN Pregnancy Case Manager will manage maternal health cases, delivering education, care coordination, and advocacy to improve health outcomes for mothers and infants.

Key responsibilities

• Coordinate care for maternal members with high needs, focusing on education and navigation of the health system
• Identify gaps in care and implement strategies to reduce pregnancy-related complications
• Collaborate with healthcare providers to ensure comprehensive support for patients throughout their pregnancy journey

Required qualifications

• Associates or Bachelor's degree in Nursing
• 5+ years of clinical/acute care experience, including direct maternal patient care
• Current, unrestricted RN license in the state of residence
• Certified Case Manager (CCM) preferred or commitment to obtain within two years of hire
