RN Referral Analyst

Job Summary

Supporting the case management team, the full-time RN Referral Analyst will monitor and review referrals, assess cases for medical management, and identify alternative outreach plans for members while working remotely.

Key responsibilities

• Monitor and review referrals from various sources to assess and assign cases for medical case management
• Review clinical information and ensure necessary documentation is attached to referred cases
• Identify alternate care outreach plans for members when referrals are not appropriate for full case management

Required qualifications

• 5+ years of clinical nursing and patient assessment experience
• Related BSN degree or equivalent work experience
• Graduate of an accredited school of nursing
• RN - Registered Nurse - State Licensure
• ICU experience with higher-acuity patients or extensive experience in medical-surgical units
