RN Supportive Care Case Manager

Job Summary

A company is looking for a Supportive Care Case Manager RN - REMOTE.

Key Responsibilities

• Coordinate care for patients with complex needs, ensuring alignment with their treatment goals
• Facilitate communication between patients, families, and the healthcare team to support care transitions
• Assist patients in developing advance care plans and navigating available resources

Required Qualifications

• Minimum of 2+ years experience as an RN in Hospice or Palliative medicine
• Minimum of five (5) years experience as an RN
• Current RN License required; BSN preferred
• Current CPR or BLS certification
• Intermediate computer skills and proficiency in MS Word, Excel & Outlook required
