RN Telehealth Triage

Job Summary

A company is looking for a Telehealth Triage RN for the 2nd Shift.

Key Responsibilities

• Perform clinical triage to assess member health concerns and direct care appropriately
• Engage members in clinical discussions to understand their needs and guide them in care decisions
• Coordinate care, including scheduling appointments and follow-up for preventative and chronic care

Required Qualifications

• RN, ADN, or BSN degree
• Active RN license in a compact state
• 5+ years of direct patient care experience, preferably in family medicine
• Triage nursing experience desired
• Experience in population health or disease management required
