Job Summary

To support the Medical Management department, the full-time RN Telephonic Care Manager will coordinate care and provide health education for adult Health Plan members through telephonic collaboration, primarily working remotely during standard daylight hours.

Key responsibilities:
  • Conduct comprehensive assessments of members' medical, behavioral, and social needs, and develop individualized care plans
  • Engage members to close gaps in preventive care and facilitate scheduling of necessary screenings or tests
  • Document all care management activities and identify trends for improvement based on member interactions
Required qualifications:
  • Minimum of 2 years of experience in a clinical setting and case management nursing
  • BSN preferred
  • Registered Nurse (RN) licensure required
  • Case management certification or approved clinical certification preferred
  • Ability to work in a designated private workspace and meet internet connectivity requirements
