RN Telephonic Case Manager

Job Summary

Supporting a diverse member population, the full-time RN Telephonic Case Manager in Transplant Medicine will identify, coordinate, and provide appropriate levels of care remotely, focusing on assessing health status, addressing treatment gaps, and delivering patient education.

Key responsibilities:

• Make outbound and receive inbound calls to assess members' current health status
• Identify gaps or barriers in treatment plans and provide patient education for self-management
• Coordinate care and make referrals to outside resources while maintaining communication with members

Required qualifications:

• Current, unrestricted Compact RN license in the state of residence
• 3+ years of experience in a hospital, acute care, or direct care setting
• Proficiency in Microsoft Office Tools and Systems (Outlook, Word, Excel, Teams)
• Access to high-speed internet (DSL or Cable) and a dedicated work area for privacy
