RN Transitions of Care Manager

Job Summary

A company is looking for a Nurse Care Manager, Transitions of Care.

Key Responsibilities

• Manage overall care management of a patient panel, leveraging nursing expertise and value-based care operations
• Establish trusting relationships with patients and families, providing clinical and emotional support
• Coordinate interdisciplinary care plans and ensure timely follow-up appointments and referrals

Required Qualifications

• Associate Nursing degree with an active RN license (compact license preferred)
• 3+ years of care management experience
• 2+ years of Transitions of Care experience
• Experience in chronic and complex care management
• Intermediate skills with MS Office Suite, including Outlook and Teams
