RN Transplant Case Manager

Job Summary

A company is looking for a Transplant Case Manager III to provide member-centered care and coordinate services for individuals with complex illnesses.

Key Responsibilities

• Ensure all members and providers are fully aware of the plan of care
• Engage members and care teams in care plan discussions and establish accountabilities
• Design and implement targeted programs to support member needs and goals

Required Qualifications

• Associate's or Bachelor's degree in Nursing
• 5+ years of clinical/acute care experience
• Advanced experience in targeted transplant programs
• Current, unrestricted RN license in the state of residence
• Certified Case Manager (CCM) preferred or ability to obtain within two years of hire
