RPM CCM Care Coordinator

Job Summary

A company is looking for an RPM/CCM Care Coordinator.

Key Responsibilities:

• Onboard new practices and create care plans for patients
• Manage a team of case managers and delegate RPM and CCM-related tasks
• Act as the main point of contact between the company and the practice provider

Required Qualifications:

• Bachelor of Science in Nursing (BSN) or Masters in Nursing
• 3+ years of clinical experience with direct patient care
• Certification in Case Management (CCM) preferred
• Experience in case management/coordination of programs and services preferred
• Trustworthy and accountable behavior, capable of maintaining confidential information
