Senior Care Consultant RN

Job Summary

A company is looking for a Care Consultant Sr for a remote healthcare contract position.

Key Responsibilities:

• Analyze practice-specific data for cost and quality improvement opportunities
• Assist practices in targeting high-risk members and addressing care gaps
• Support providers in implementing population health management strategies

Required Qualifications:

• Current active unrestricted license as RN, LCSW, LMSW, LMHC, LPC, LBA, LMFT, or Clinical Psychologist
• BA/BS degree
• 5-10 years of healthcare experience or equivalent background
• Experience in practice transformation, quality improvement, and data analytics preferred
• Clinical background or MBA is a plus
