Job Summary

A company is looking for a Senior Care Navigator to guide clients through the behavioral health landscape and provide support in their time of need.

Key Responsibilities:
  • Conduct outreach to understand and assess higher risk clients' needs, and take action when necessary with the support of clinical staff
  • Support clients along their care journey, from entry into the system to their first appointment and beyond
  • Leverage clinical expertise to determine the most suitable level of care and provider for clients based on needs, preferences, and availability
Required Qualifications:
  • Master's level mental health/clinical degree
  • 2-3 years of experience in Care Navigation/Coordination/Intake within the mental health or healthcare arena
  • Experience with telephonic crisis intervention
  • Operational experience within fast-growth healthtech, ideal
  • Self-starter, proactive, and solution-oriented
