Job Summary

Providing support to Community Health Partners, the full-time Senior Community Health Partner will assist in integrated social care for members with complex health needs while working remotely.

Key responsibilities

• Supports Community Health Partners and Care Team Lead in daily operations
• Guides, coaches, and trains teams on engagement, care planning, and longitudinal care
• Maintains a panel of members, acting as a point person to help them achieve their goals

Required qualifications

• 3+ years of experience in care coordination, community health, health promotion, or a related field
• 6+ months of experience managing or leading people
• Bachelor's degree
