Key Responsibilities

• Oversee the execution of ECM, Community Supports, and CHW programs across various locations
• Manage team performance and ensure adherence to defined workflows and operational consistency
• Identify and resolve operational issues affecting care delivery and escalate as necessary

Required Qualifications

• Direct experience with CalAIM programs in California
• 10-12+ years in healthcare operations focused on care delivery or population health
• 5+ years of experience managing multi-layered teams in a distributed environment
• Proven track record in program execution and performance management
• Strong understanding of Medicaid populations and care coordination workflows
