Senior Living Care Navigator

Job Summary

A company is looking for a Senior Living Care Navigator.

Key Responsibilities:

• Conduct telephonic outreach to members and assist with care coordination
• Guide members on utilizing plan benefits and complete telephonic assessments
• Facilitate communication between members and the care team, providing community resource information

Required Qualifications:

• High School Diploma / GED or equivalent work experience
• Must be 18 years of age or older
• 2+ years of experience in a healthcare setting with patient and medical team communication
• Experience working in cross-functional, inter and multi-disciplinary teams
• Proficiency with EMRs and knowledge of medical terminology
