Perform telephonic, SDOH, and RSA assessments

Required Qualifications

High School Diploma/GED (required)
2+ years' experience in a physician office, SNF, ALF, or outpatient setting
CNA, CMA, or LPN background preferred
Proficiency in Microsoft Word, Excel, and Outlook
Available Monday-Friday
