Job Summary

A company is looking for a Senior Population Health Strategy Specialist to plan and design population health strategy initiatives and programs.

Key Responsibilities

• Develops and designs complex population health strategy initiatives and operational plans to improve care coordination and health outcomes
• Monitors and researches care management trends and analytics to support program initiatives and identify high-risk member populations
• Tracks and monitors program participation and member outcomes to assess the efficacy of programs and make recommendations for improvement

Required Qualifications

• Bachelor's degree required
• 4 - 6 years of related experience required
• Equivalent experience may be considered in lieu of formal education
• Experience in developing financial budgets and forecasts for strategic planning
• Knowledge of compliance with federal and state regulations and NCQA guidelines
