Senior Population Health Specialist

Job Summary

A company is looking for a Senior Population Health Strategy Specialist to plan and design initiatives aimed at improving care coordination and health outcomes.

Key Responsibilities

• Develop and design complex population health strategy initiatives and operational plans
• Monitor and research care management trends to support program initiatives and identify high-risk member populations
• Track program participation and member outcomes to assess efficacy and recommend improvements

Required Qualifications

• Bachelor's degree required
• 4 - 6 years of related experience preferred
• Equivalent experience may be considered in lieu of a degree
• Must reside in Nevada or Arizona
• Authorization to work in the U.S. without the need for employment-based visa sponsorship
