A company is looking for a Senior Population Health Strategy Specialist who will plan and design population health strategy initiatives to improve care coordination and health outcomes.

Key Responsibilities

• Develops and designs complex population health strategy initiatives and operational plans
• Tracks clinical, quality, documentation, and data submission projects to achieve strategic objectives
• Monitors care management trends and analytics to support program initiatives and identify high-risk member populations

Required Qualifications

• Bachelor's degree required
• 4 - 6 years of related experience required
• Equivalent experience may be considered
• Must reside in Indiana or Missouri
• Authorized to work in the U.S. without the need for employment-based visa sponsorship
