Service Coordinator I

Job Summary

A company is looking for a Service Coordinator I to assist with client intake and care coordination.

Key Responsibilities

• Maintain and coordinate a caseload of clients enrolled in Medicaid programs
• Coordinate clients' in-home care needs in compliance with regulations and program rules
• Assist with daily intake and referral processes to promote business growth

Required Qualifications

• Bachelor's degree in Social Work, Psychology, Counseling, Nursing, Sociology, or related field preferred
• 1 year of experience working with individuals with disabilities or the elderly preferred
• Combination of education and experience may be considered
• Reliable internet access required for remote/hybrid positions
