Job Summary

A company is looking for a Skilled Inpatient Care Coordinator in Georgia for a remote position.

Key Responsibilities:

• Perform assessments on patients in skilled nursing facilities using clinical skills and CMS criteria
• Collaborate with healthcare teams to establish optimal discharge plans
• Engage with patients, families, or caregivers telephonically or on-site weekly and as needed

Required Qualifications:

• Active, unrestricted clinical license in Georgia as a Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Therapist
• 3+ years of clinical experience in one of the mentioned roles
• Proficiency in Outlook, Excel, and Word
• Dedicated, distraction-free home office space and high-speed internet access
