SNP Engagement Specialist

Job Summary

Working remotely in a full-time capacity, the SNP Engagement Specialist will coordinate care for members, ensuring compliance with healthcare regulations while supporting the clinical team in managing transitions of care and health risk assessments.

Key responsibilities

• Assist in the implementation and oversight of care management programs, ensuring compliance with CMS and NCQA standards
• Coordinate transitions of care and support members in accessing services and understanding benefits
• Conduct follow-ups with members to ensure planned services are delivered and document care needs accurately

Required qualifications

• High School diploma or equivalent
• One year of experience in a healthcare or managed care setting
• One year of customer service experience
• Knowledge of Medicare, Medicaid, or SNP populations is preferred
• Associate Degree in a related healthcare field is preferred
