Job Summary

Working remotely, the full-time Social Care Manager - Utah Licensed will coordinate care for patients by conducting psychosocial assessments, connecting members to community resources, and advocating for patients' needs while managing care plans and communication with stakeholders.

Key responsibilities

• Assess, plan, and implement individualized care strategies for patients
• Document care plans and identify community resources while working virtually
• Advocate for patients and families to ensure their needs are represented in care decisions

Required qualifications

• Bachelor's degree in social work or a related field
• 2+ years of experience in care coordination or case management
• 1+ year of clinical experience in a healthcare setting
• Experience navigating community healthcare and social services resources
• Intermediate to advanced proficiency in Microsoft Word, Excel, and Outlook
