Job Summary

A company is looking for a Social Care Navigator (Remote).

Key Responsibilities

• Conduct high-volume outreach and screening for Medicaid members to assess eligibility and identify next steps
• Utilize a virtual contact center platform to manage member interactions and document screening outcomes
• Provide accurate information and navigate members to eligible nutrition services and support resources

Qualifications

• High School Diploma, HSE, or GED required
• Experience in high-volume outbound calls, customer service, or call center environments
• Working knowledge of Medicaid and healthcare benefits preferred
• Familiarity with community resources and social service programs preferred
• Proficiency in operating computer software and technology platforms for documentation
