Job Summary
Supporting the Social Determinants of Health screening and referral programs, the full-time remote SDOH Care Coordinator will manage data entry, place e-referrals, and follow up with families to connect Medicaid members with community resources across six states.
Key responsibilities
- Enter SDOH assessment results into the Findhelp platform and verify patient Medicaid eligibility
- Place e-referrals to community-based organizations and follow up with families to confirm referral outcomes
- Coordinate with front desk staff to support screening workflows and contribute to continuous improvement efforts
Required qualifications
- Bachelor's degree in social work, public health, community health, human services, or a related field; OR certification as a Community Health Worker (CHW) with relevant professional experience
- Minimum of 2 years of experience in care coordination, community health, or social services with direct family engagement
- Demonstrated understanding of social determinants of health frameworks and their impact on pediatric populations
- Proficiency with web-based platforms and data entry, with a comfort in learning new technology tools
- Ability to work independently in a fully remote setting, managing priorities across multiple states and time zones
