Job Summary
A company is looking for a Social Service Specialist to facilitate appointments and resources for members qualifying for Long Term Care services.
Key Responsibilities
- Facilitates appointments and clarifies eligibility data for Long Term Care resources
- Submits applications and follows up with government agencies on claim statuses
- Maintains accurate documentation and communicates effectively with clients and agencies
Required Qualifications
- Minimum of one year of experience as a Case Manager or similar role
- At least one year of healthcare experience preferred
- Basic knowledge of Medicaid/Medicare and Long Term Care
- High School Diploma or GED required; Associate's degree preferred
- Experience in a Customer Service Center and familiarity with phone systems
