Job Summary

A company is looking for a Social Service Specialist to facilitate appointments and resources for members qualifying for Long Term Care services.

Key Responsibilities
  • Facilitates appointments and clarifies eligibility data for Long Term Care resources
  • Submits applications and follows up with government agencies on claim statuses
  • Maintains accurate documentation and communicates effectively with clients and agencies
Required Qualifications
  • Minimum of one year of experience as a Case Manager or similar role
  • At least one year of healthcare experience preferred
  • Basic knowledge of Medicaid/Medicare and Long Term Care
  • High School Diploma or GED required; Associate's degree preferred
  • Experience in a Customer Service Center and familiarity with phone systems
