Social Service Specialist

Job Summary

Working remotely on a part-time basis, the Social Service Specialist will engage with client members via phone to assess and address their needs related to Social Determinants of Health (SDOH), provide information on benefits eligibility, and facilitate access to community resources.

Key responsibilities

• Respond to calls from members/patients seeking assistance with benefits eligibility and community social services
• Conduct standardized assessments to identify member needs and eligibility for resources related to SDOH
• Document all member interactions and maintain accurate records in the Centauri Health Solutions database

Required qualifications

• High School Diploma or GED equivalent
• 1+ years of experience in a professional office and customer service environment
• At least one year of experience in social services or healthcare preferred
• Proficient in MS Office and Windows operating systems
• Basic knowledge of social services and issues faced by underserved populations
