Job Summary

A company is looking for a Coordinator - Social Services.

Key Responsibilities

• Obtain and share information on benefits and community resources to support patients after discharge
• Assist in sending Medicaid applications and other necessary documentation for patient benefits
• Collaborate with nurse coaches to ensure patients have access to community resources for recovery

Required Qualifications

• Degree in social work or a related field
• Deep knowledge of community amenities and resources
• Understanding of medical terminology and planning
• Experience in recognizing Health Plan benefits and community resource needs
• 5+ years of experience in community resource management or Health Plan benefit management
