Job Summary

A company is looking for a Coordinator - Social Services.

Key Responsibilities
  • Obtain and share information on benefits and community resources for patients post-discharge
  • Assist in processing Medicaid applications and other necessary documentation for patient benefits
  • Collaborate with nurse coaches to provide patients with information on community resources for recovery support
Required Qualifications, Training, and Education
  • Degree in social work or a related field
  • Deep knowledge of community amenities and resources
  • Strong understanding of medical terminology and planning
  • Experience in recognizing Health Plan benefits and community resource needs
  • 5+ years of experience in community resource management or Health Plan benefit management
