Job Summary
Providing member-centric care, the full-time Social Work Case Manager - State Licensed will conduct individualized assessments, coordinate services, and connect members with community resources while working remotely.
Key responsibilities
- Manage a portfolio of clients with high needs, facilitating access to appropriate health services
- Collaborate with members, families, and interdisciplinary teams to develop and implement care plans
- Utilize evidence-based practices to reduce the burden of illness and improve overall health outcomes
Required qualifications
- Bachelor's degree in Social Work; Master's degree preferred
- 5+ years of clinical/acute care experience
- Current, unrestricted Social Work license/certification in the state of residence, with preference for clinical licensure
- Certified Case Manager (CCM) preferred, or ability to obtain within two years of hire
- Experience working with vulnerable and complex populations in a clinical or home care environment
