Job Summary

A company is looking for a Community Health Worker (Remote - Upstate Region).

Key Responsibilities

• Provide guidance and support for members through home visits and care coordination
• Assist with member engagement, onboarding, and facilitate referrals to clinical care programs
• Support community outreach and promote programs through participation in community events

Required Qualifications, Training, and Education

• High School Diploma or equivalent
• 3 years of work experience, including 1 year of community involvement or customer service experience
• Understanding of basic principles of Medicaid programs and knowledge of community resources
• Community Health Worker certification within 6 months of hire
• Proficiency in Microsoft Office
