Job Summary

A company is looking for a PCMH CMA Care Coordinator.

Key Responsibilities

• Identify patients qualifying for care coordination and arrange follow-up services
• Communicate effectively with patients and care teams to support continuity of care
• Identify patient needs and coordinate contact with community resources

Required Qualifications, Training, and Education

• 5 years of relevant medical office experience required
• CMA certification from an accredited body is required
• Current Basic Life Support (BLS) certification is required
• Experience with Epic registries and reports is preferred
• Ability to maintain accurate data and documentation
