Job Summary

A company is looking for a Telephonic Behavioral Health Caseworker.

Key Responsibilities

• Manage and coordinate care for Members with behavioral health conditions and complex needs
• Support Members during transitions of care and ensure medication reconciliation
• Conduct telephonic visits for education and assessment, maintaining accurate documentation

Required Qualifications

• Bachelor's Degree in Human Services or related field; Master's preferred
• 2 years of experience working with Serious Mental Illness (SMI) members
• 2 years of social services experience engaging with clients and assessing needs
• Proficiency in MS Office and electronic medical record documentation
• Ability to manage a caseload of up to 60 Members and support care transitions
