Job Summary

A company is looking for a Care Coordinator to manage patient care and health coaching remotely.

Key Responsibilities

• Manage a panel of 400-450 patients, conducting monthly outreach and daily calls for care plan support
• Collaborate with clinical teams to address patient needs and support care coordination goals
• Document patient interactions in real-time while maintaining HIPAA compliance

Required Qualifications

• Must hold a nationally recognized medical assistant certification or be a Certified Pharmacy Technician
• Minimum 2 years of clinical experience in a healthcare setting
• Experience with Electronic Medical Records (EMRs) and strong computer navigation skills
• HIPAA-compliant home office setup with minimum internet speed requirements
• Must be legally authorized to work in the United States
