South Carolina Licensed Care Coordinator

Job Summary

South Carolina Licensed Care Coordinator, this full-time remote position is responsible for reviewing and evaluating medical or behavioral eligibility for benefits, coordinating care plans, and providing health management interventions to support members in managing their health.

Key Responsibilities

• Perform medical or behavioral review/authorization processes and ensure coverage for appropriate services
• Utilize resources to support review determinations and participate in data collection for clinical information flow
• Engage in direct intervention and patient education regarding healthcare delivery systems and care management programs

Required Qualifications, Training, and Education

• Associate Degree in Nursing or a Master's degree in Social Work, Psychology, or Counseling
• 2 years of clinical experience
• Active, unrestricted RN licensure or LMSW licensure in the state of hire
• Working knowledge of Microsoft Office
• Ability to work independently and prioritize effectively
