South Carolina Licensed Care Manager

Job Summary

A company is looking for a Care Manager (RN) to develop and facilitate care management activities for members with physical health needs.

Key Responsibilities

• Evaluate member needs and recommend care plans to address barriers to accessing appropriate services
• Coordinate care among members, families, and providers to ensure person-centered services
• Monitor member progress and update care plans as necessary to meet individual goals

Required Qualifications

• Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing
• 2 - 4 years of related experience in care management
• Registered Nurse (RN) state licensure or Compact State Licensure required
• Must reside in South Carolina
